Our Patient Portal is a secure, HIPAA compliant website that allows you to access your previous visit records and results from our office. You can also update your demographics, health history information, and complete patient forms.

About Your Visit
Our doctors and staff look forward to meeting you! We respect your time and would like to make your visit to our office as efficient as possible.
NEW PATIENTS: If this is your first visit to our office, we have included our New Patient paperwork below:
Note: The Authorization form allows you to specify other people with whom we can share your information, such as a spouse, child, relative, or friend.
Other items to bring with you may include:
- Insurance cards
- Referral , if your insurance requires it
- List of allergies
- List of your current medications
- Pertinent records from your physician (recent bloodwork or other testing)
- CT scans or MRI films
- Any recent audiograms/hearing tests
- Past surgical history
If you are bringing in a child under 18
We require that the parent(s) or guardian(s) (with appropriate paperwork) come with their child on their first visit as well as any visit pertaining to potential surgery. Failure to do so will result in rescheduling the appointment.
If your child is an established patient with non-surgical issues or concerns and you plan to have a grandparent, other relative, etc., bring your child in for their visit, please print and fill out the form below. Without this form, we will be unable to see your child.
Appointment Reminders
Our office utilizes an appointment reminder service as a courtesy to notify you of your upcoming appointments. We can reach you via text message, email and home phone. Please let us know if you have a preference for reminders so that we can note your account. Please be aware that failure to notify us 24 hours in advance could result in a rebooking fee.
Financial Policy
We will collect your deductible, co-payment, or charges for non-covered services at the time of your visit. Even though an insurance claim may be filed, you are responsible for the total amount on your account. if you have a balance after an insurance payment from a previous service, we will also ask for that payment. We accept cash, checks, debit, and credit cards.
Cancellation Policy
If you are unable to keep your appointment, we ask the courtesy of a phone call to (810) 694-0600 as early as possible, so we are able to offer that time to another patient. If you do not call 24 hours in advance and do not show for your appointment, there will be a $50 fee to reschedule.
Medical Records
Please fill out the appropriate form and fax to 810-603-3152 or email to [email protected] (non-secured email).
- Medical Records Request – this form allows you to get a copy of your medical records for personal use.
- Medical Records Release – this gives us your permission to send your records to another physician
Please note: If you have an attorney who would like to obtain a copy of your records, they must contact us directly. An Authorization to Release Records must be signed by you in advance.
About Your Surgery
Surgery can often be an overwhelming decision to make, not only in regards to your health but financially as well. We strive to make our patients feel as comfortable as possible in all aspects of surgery scheduling.
When you sit down to discuss surgery, we will help you:
- Schedule your procedure at a time that is convenient for your and our doctor
- Plan financially for any costs you might incur by discussing your insurance benefits, out of pocket costs and any additional charges you can expect to incur based on the procedure you have done
- Understand what to expect before, during and after surgery
- Schedule a Pre-Op appointment (if necessary) as well as a Post-Op appointment
- About Your Surgery
The time of your surgery may vary and you will be notified by the hospital/surgery center in advance to confirm the time you need to arrive – usually they will call the day before your surgery. Typically, younger patients are scheduled to go first. If you need to cancel or postpone your surgery for any reason, we ask that you call our office at least 72 hours in advance of your surgery date to avoid a cancellation fee of $50.
If you have an insurance deductible to meet, please be prepared to pay this when you schedule surgery. This payment is expected at that time.
